Terrible and unsafe environment
My family member walked into this facility with a walker on [date removed]. Three weeks later she became unable to walk or could only stand with assistance. Her right foot was injured and required an x-ray. She also fell out of bed and her daughter was notified by an attendant. Later the Director made an incident report. The patients there spend an inordinate amount of time in a room where the TV is on, however, many to do have the cognitive ability to engage mentally in a TV show. There is very little emphasis on Individualized Memory Care. The food served is not a balanced diet. The Director is a Licensed Vocational Nurse who is very controlling, and she does not possess the qualifications or knowledge to adequately operate a memory care facility. They offer incentives to bait you in but the care very poor and a high staff turnover. We had to move our family member to keep her safe. I have witnessed patients there with black eyes from falling.

Excellent Care
Autumn Leaves was great. During the short time that my loved one was there, they took very good care of him. I was very pleased. It's a very nice place, and the people were just wonderful. They were very good to him and to us during his time on hospice. A couple of the caregivers in particular were outstanding, and I appreciated their support so much. I would recommend Autumn Leaves to anybody with a loved one in need of care.

A Good Place
You have to watch your family member - don't just drop them off. I think that would be the case at any care facility. You need to be active in monitoring your loved one. Be sure to understand what's going on with medications. They have an in-house nurse practitioner, who they contacted for additional medications without contacting me. The nurse practitioner called me about, but Autumn Leaves didn't. I think the day to day staff that helps my mom has been great. But I'm not so sure about management.

This is an excellent place to go and have your loved ones at. The professionalism of the staff is wonderful. The staff communication with the family is excellent. The meals are good, and there are three meals a day served to the residents. There are also snacks served daily. The community can accommodate some dietary restrictions.

Happy With My Choice - Would Recommend
I took my husband out of a different facility because I was very unhappy with it (understaffed for the amount of patients, theft, etc.). Moving into Autumn Leave, it has been absolutely fine. He's getting very good care. I like how clean the facility is. I'm very happy to have him here (as happy as you can be considering the circumstances). Autumn Leaves is also cheaper than the previous place, which was in no way the reason for my decision, but a nice bonus feature.

Wonderful Choice as a Memory Care Facility
The staff are always very helpful and keep me advised of how how my loved one is dong in general, even when things are going well. They're good at keeping me in the loop. He tells me the food is good. When you walk in, it's like you're walking into a very large home, with sofas and cozy seating areas around a fireplace. They have music playing. It's a very home-like environment.

A great place for MOM!
We had to make the decision to move my MIL into a memory care facility. The level of care that helped us during the transition has been great! I manage many employees for my work and I can honestly say that the employees that Autumn Leaves currently have are fantastic! They truly care about the residents and their family members! I truly feel that I have found a great place for MOM!

I recommend Autumn Leaves Memory Care for anybody looking for a place for their family member in Spring TX. Autumn Leaves, Spring TX have some of the most caring staff in the facility along with a beautiful set up for memory care clientele. [name removed] the Director of Sales and Marketing has a wealth of knowledge that she has brought to Autumn Leaves and shares with the families and the community!!!

We love it here!
We had so many options when looking for dementia care for our mother so we were a little overwhelmed. But when we toured and ended up meeting with [name removed] the Executive Director she helped put us as ease with her nurturing style and expertise on all things dementia care. Our mother has been there for a year now and we couldn’t be happier. Don’t hesitate to call here because between all the activities they do and kind, caring staff we feel Mom is in safest, best staffed place in town.

Very happy here
We weren't sure where to find a place for our mother who 91 and has dementia. And there are SO many of them in the area. But after taking one of those virtual tours we saw how upbeat and friendly the staff were, and they like had tons of different areas for activities so we knew this was the right choice. She has been there for 2 months now and it such a relief knowing she is a home that really. I HIGHLY recommend Autumn Leaves.

God's angels on earth
This has got to be the best memory care facility out there. [Name removed] and her staff really know how to care for the residents. They truly are caring and nurturing. I see how they interact with the residents when "no one is looking" and it just warms my heart to know my dad is receiving the same attention when we're not there. RUN AWAY
Most of the staff seems to care about the people in this facility. However, the management is neglectful and uncaring and hard on the staff. The management creates a miserable environment and a high turnover rate. Within 2 weeks of living at this facility, my grandmother fell and broke her hip. I understand that accidents happen and old folks fall however while living here she also received multiple black eyes and severe had bruising on her forehead. I have photos and they would best tell this story but that would be an invasion of my grandmother's privacy and I wouldn't want to do that to her. I have witnessed this facility force opioids on my grandmother despite her refusal verbally and her spitting out the medication. Other times she would be so overly medicated she could not consent to receive the medication and they would just give it to her anyway with a syringe. Multiple staff members told me if they did not do this they would be reprimanded by management. They did not allow my grandmother to maintain any dignity whatsoever. Luckily my grandmother was removed from this facility several months ago has been living just fine for over a year without being overly medicated and with her dignity intact. All of this- despite the Autumn Leaves and hospice claiming she was going to die within a week just before we removed her and took her to the hospital. Methodist hospital diagnosed her with either " being poisoned or being extremely over medicated". I also advise caution when selecting a hospice company I would go with a nonprofit one - do not choose [Name removed]. Do Not put your loved one in Autumn Leaves- do more research - there are other options. Even if you are just trying to find a place to put a difficult relative so you don't have to worry about them anymore - trust me - no one deserves this place. You don't have to choose between an extremely expensive facility or a state-run facility. There are nonprofit facilities which have a more humane approach to caring for the elderly - even those with memory loss. My grandmother is now at a different facility and although the food is terrible- the staff is impeccable the doctors are wonderfully attentive and the management has been fantastic and compassionate. And they are willing to feed her the food that I bring her. Although I don't wish memory loss on anyone, I am grateful that my grandmother doesn't remember her time at Autumn Leaves of Cypresswood. When visiting these kinds of facilities make sure that you stay overnight several times randomly so that you get a full understanding of the environment especially at Autumn Leaves of Cypress Wood. Do not just visit for 15 minutes and think you're going to get the whole picture- there is a lot going on under the surface. If your loved one is not going to harm another person or runaway I do not advise putting them in Autumn Leaves ever. If they just have some memory problems you definitely have other options And honestly even if they are harming themselves or others- just look elsewhere - as I wrote before- NO ONE deserves this place.

They Are Trying their Best
They are trying very hard to do what they can for my wife. She's having a hard time adjusting. However, I feel they could be proactive on feeding her and the meals could be better tasting. The staff call me with updates about how she is doing. It is clean from what I can tell but the furniture could be cleaned or replaced. I piked the community because they seemed more like a memory care.

When I have been in Autumn Leaves several times. I am not impressed on what I have seen. The people are sitting in a circle and looked drugged out placed in front of a t.v. Seems that there is no life in that facility and is DEPRESSING.
